Benefit Plans & Payment options:

 

We’re here to make your visits as easy as possible. We are in network with most dental insurance plans, including the Microsoft employee dental benefit plan. Do you have a question regarding your dental benefits? Give us call, we’re happy to walk you through it.

 

We are a preferred provider with the following benefit plans:

  • Regence
  • Premera
  • Washington Dental Service-Delta Dental
  • Cigna
    Teachers-We are in network with the Uniform or Standard plan

If you currently do not have dental coverage, we can help you make dental treatment affordable with low or no interest financing. Cash, credit, and checks accepted. Cash and senior citizen discounts available.

Financial Policy

All dental services must be paid for at the time services are performed unless other arrangements are made. All dental services are charged directly to the patient and the patient is personally responsible for payment. A service charge of 1.5% per month (18% per annum) on the unpaid balance will be charged on all accounts exceeding 60 days. Charges for services shall be as billed unless objected to, in writing, within the time payment is due.  Fee estimates for dental care can only be extended for a period of six months from the date of the treatment plan. Any accounts over 90 days with a balance due and without any outstanding insurance claims may be transferred to a collection agency. Patients will be notified in writing if their account is pending transfer. There will be a fee of $35.00 assessed to all accounts for returned checks.

Insurance and Authorization

Insurance carriers can be billed directly and Emma Etemadi, DDS, PS will accept assignment of payment. Patients agree to pay deductibles, estimated coinsurance amounts, and non-covered fees, as determined by their insurance carrier.

Cherry Valley Dental can help prepare insurance forms or assist in making collections from insurance companies and will subsequently credit any collections to the patient's account. However, Emma Etemadi DDS PS and Cherry Valley Dental cannot render services on the assumption that charges will be paid by an insurance company. Patients authorize Emma Etemadi, DDS, PS to release any and all information needed exclusively for the payment of professional charges and to permit representatives of those responsible for such payment the examination and copies of all records relating to the care and treatment received, if requested. It is the responsibility of the patient to notify Cherry Valley Dental of any changes in insurance coverage.
